How Do I Submit A New Referral to LIVEWELL


We’re honored to be considered as a care partner for your client.

To submit a referral, please follow the steps below using our secure online form.


Step-by-Step Instructions

  1. Visit www.lwhcare.com

  2. Click the “Referral” link in the top right corner.

  3. After navigating to the form, Enter your contact details and select “I’m a Service Coordinator.”

  4. Under “What can we help you with?” choose “Make a Referral.”

  5. Complete the required fields:

    • Select your Regional Center

    • Enter the client’s name and contact information

    • Identify the Service/Support type requested 

      • Livewell is vendored for In Home Respite (862) and Personal Assistance Services (062)

    • Indicate if this is for an Agency Provider or Family-Selected Provider

      • If Family-Selected, include the care provider’s name and contact information

    • Use the Additional Information box to include key context (see tips below)

    • Use the "Attach a file" link to provide the client’s most recent IPP (if available) and any other relevant documents


What to Include in the Additional Information Section

Providing details here helps us process your referral more efficiently:

  • Preferred method of communication with family

  • Language or cultural preferences

  • Behavioral/medical considerations

  • Intended start date 

  • Family/home setting notes

  • AND any other information we should be aware of to help ensure timely processing
