What Happens After A Referral Is Made

Once a referral is submitted to LIVEWELL, our team begins processing right away. The next steps depend on whether a Family-Selected Care Partner or an Agency-Selected Care Partner has been requested.


Family-Selected Care Partner

Step 1: LIVEWELL will contact the family to confirm the referral and provide the application packet to the selected care provider.

Step 2: The care provider must complete all onboarding steps, which may include:

  • Submitting ID and eligibility-to-work documents

  • CPR certification (Respite only)

  • Completing Mandated Reporter training

  • Undergoing background clearance and reference checks

Step 3: Once onboarding is complete, LIVEWELL will notify the Service Coordinator and request the Purchase of Service (POS) be generated.

Step 4: Once the POS is received, the Care Partner may begin services.


Agency-Selected Care Partner

Step 1: The LIVEWELL Staffing Team will contact the family to gather care preferences, availability, and support needs.

Step 2: Our team will match the family with a qualified provider and schedule a Meet & Greet to ensure compatibility.

Step 3: If it’s a good fit, LIVEWELL will notify the Service Coordinator and request the POS be generated.

Step 4: Once authorized, services may begin.
