If You’re No Longer Comfortable with Your Care Partner

You have the right to:

  • End services with your current provider

  • Request a replacement (if using an agency-selected Care Partner)

  • Pause services temporarily until a new plan is in place

We understand that dynamics can shift, and it’s okay to speak up if things feel off.


What to Do Next

Please submit a Support Ticket at www.lwhcare.com under the Family Support section. In your message, let us know:

  • That you’d like to discontinue care with your current provider

  • Whether you’re seeking a replacement or wish to pause services for now

  • Any context you’re comfortable sharing (this helps us serve you better)

We will never retaliate or penalize families for voicing concerns.


If Your Care Partner Is a Family Member

It can feel especially hard to make a change when your provider is a relative or friend. Please know:

  • You are not obligated to continue care if it’s not working

  • Our team can step in with neutral communication and help coordinate a respectful offboarding

  • Let us know if you have a replacement in mind or we can begin searching for an agency-based provider if desired
