Your Service Coordinator (SC) – Regional Center Staff
Your SC is your main point of contact for Regional Center services. Their role includes:
Developing and updating your Individual Program Plan (IPP)
Authorizing hours for services like Respite and Personal Assistance
Requesting new or additional services
Connecting you to vendors (like LIVEWELL) who can provide those services
Approving schedule increases or changes in your care plan
If you need more hours, want to update your IPP, or add new services, your SC is the right person to contact.
LIVEWELL – Your Service Provider
LIVEWELL is a non-medical home care agency and an authorized vendor through your Regional Center. Once services are authorized, LIVEWELL supports you by:
Helping you start services or onboard a family member as a provider
Matching and assigning agency-selected Care Partners
Scheduling shifts for agency providers
Providing support and training for Care Partners
Maintaining compliance, timesheet review, and EVV tracking
